Hi, This is G. Nandini a fifth semester medical student. This is an e-LOG depicting patient's de-identified data centered approach for learning medicine. This log has been created after taking consent from patient and his family. Here we discuss about patient's problems with a series of inputs with an aim to solve them.    DOA: 28/07/2022 CHIEF COMPLAINTS: A 49 yr old male resident of cherlapally came to the opd with the chief complaints of pain in the right lower leg since 20 days and fever since a week . Here we discuss about patient's problems with a series of inputs with an aim to solve them.    DOA:18/07/2022 A 55 year old female  agricultural worker by occupation (stopped 2 years ago) came with cheif complaints of yellowish discolouration of eyes and body , Fever and generalized weakness since 20 days .   HISTORY OF PRESENT ILLNESS: Patient was apparently asymptomatic 20 days back and developed fever associated with burning Micturtion and they also noticed yellowish discolouration of eyes and skin and bought her to our hospital .
